Citizens Advice Mid Mercia is pleased to collaboratively work with Joined Up Care Derbyshire to deliver a new pilot service for 12 months (Crisis Inreach) which will provide preventative care and support for people with a learning disability & autistic people (people with LD&/ASC) who are at imminent risk of: family or carer breakdown; harm to themselves or others; and/or inpatient admission
The Crisis inreach Service would like to recruit 8 (4 Lead Crisis Inreach Workers and 4 Crisis Workers) enthusiastic, friendly, and capable inreach workers to provide support to people with a learning disability and / or autistic people in their daily lives and could include; helping to tidy and clean the home, provide basic care for any pets (such as feeding and walking), engage in home-based activities of interest (such as watching TV, reading, playing games, and so on), hold empathetic conversations and more.
The aim of this service is to provide crisis inreach support for people with LD&/ASC who, at any time, are at risk of otherwise being admitted to a mental health inpatient setting.
We are looking to recruit a diverse mix of male and females with various skillsets, backgrounds and languages so they can be matched as accurately as possible to individuals referred and their presenting needs.
